Review templates changed

Symptom : The details of a review template keep changing. For example, information from descriptions changes and default behavior changes, such as the list of default reviewers or approvers.

Resolution: Save review templates with unique names. Ensure that the name you provide in the <template_name> element is unique. If you do not change the name, existing templates with the name value in the <template_name> element are replaced.
